<span style="color:rgb(255, 0, 0)">that you complete this task prior to 9/1/2011.</span><br>

co-investigator, and any other person at the University who is
responsible for the design, conduct, or reporting of research or

should read the policy, complete the form and download it for
signatures. When signatures are obtained, the COI can be scanned and
uploaded by the designated person in your Department into the Division
of Research database, RD2K. Instructions for uploading are also posted

<p class="MsoNormal"> </p><p class="MsoNormal"><u>COI forms that report any conflicts must include an explanation of the conflict and before they are scanned and uploaded</u>. <span style="color:rgb(255, 0, 0)">Anne Sherman must be notified and sent a COI form with a possible conflict is submitted</span>
and she will follow-up with the Vice President for Research. If a COI
requires signature by the UH VPR, a copy will be scanned and uploaded
by DOR staff.</p>

seeking a cost-share (mandatory or voluntary) and/or IDC reduction

least two weeks must be allowed for cost share and reduced IDC issues to be
resolved and documented.</span></p>
