[CCoE Notice] FINAL Reminder: New UH COIC Forms

REMINDER: YOU WILL NOT BE ABLE TO SUBMIT A PROPOSAL EITHER AS A PI OR A
CO_PI IF YOUR UH COIC IS NOT IN THE RAMP SYSTEM ON 10/1/2012.
All:
The University of Houston has made significant changes to its Policy on
Financial Conflicts of Interest in response to new regulations promulgated
by the United States Department of Health and Human Services. The updated
forms are attached.
Some additional Notes:
- Approval Process: It’s best for PI's to monitor this process
through to completion and keep a copy of their approved COIC on their
desktop. If a form does get lost or is not uploaded for some reason, the
PI can always send a desktop copy to Gerri, Rudy, and Winsome directly so
there is no delay.
- Upload of COI to RAMP: Once the Department receives a completed
form, the COIC needs to be uploaded immediately to ensure submissions and
award setups are not delayed.
- RA Group: We will keep copies of the COIs sent to us. During
the transition, the PI must send us a PDF of the approved form if not
already visible in RAMP. If you prefer, we can upload to RAMP. This
means that the Department will need to check before uploading to ensure two
duplicate COIs are not in RAMP.
- Disclosure of Financial Interests: If you see this situation,
please let me know so I can be sure the associated information is uploaded.
*All forms, whether a conflict exists or not, must be signed by the
Department Chair and Dean*.
Faculty should read the policy, complete the form and download it for
signatures. When signatures are obtained, the COI can be scanned and
uploaded by the designated person in your Department into the Division of
Research database, RD2K.
Additionally, as a reminder, please re-familiarize yourself with our
Pre-Award submission process:

Pre-Award Grant Submission Process *
1. When considering a submission of a grant/contract proposal, the PI
will notify the appropriate Research Administrator (RA) four weeks in
advance, if possible.
2. When constructing the UH “transmittal,” if the administering unit
for the grant reports to the Dean of Engineering, RAs Hamlett, Chee,
and/or Razo will assist with the proposal submission (pre-award).
3. When seeking a cost-share (mandatory or voluntary) and/or IDC
reduction (sponsored mandated and/or voluntary), the request should be
channeled through the appropriate Department Chair. At least two weeks
must be allowed for cost share and reduced IDC issues to be resolved and
documented.
4. * **A current UH Conflict of Interest Form must be on file (for PIs
and co-PIs) before UH will submit a grant/contract proposal to an external
sponsor.*
