Disregard if you are already aware of the UHS FERPA release that is personalized for reference requests from students and former students.

The link to the University of Houston System Reference Request & FERPA Release form is below:

In my opinion, filling out the above form is much more straightforward for reference requests than populating the general Authorization to Release Educational Records form available at the link below:
